Earlier, endovascular surgeons of the Pediatric University performed stenting of the excretory part of the right ventricle of the heart. This technique was used by doctors for the first time in the North-Western Federal District. Four months later, the heart surgeons of SPbSPMU successfully completed the second stage of treatment. The open-heart operation with artificial blood circulation lasted 3.5 hours.

The tetralogy of Fallot is a group of cardiovascular abnormalities that belongs to the cyanotic (blue) malformations. Patients have a defect of the interventricular septum – an opening through which non-oxygen-rich blood enters the aorta. Correction of the defect of severe forms of this defect in small children with shortness of breath attacks is usually carried out in two stages: initially, it is necessary to increase the blood flow to the pulmonary artery so that the patient does not suffer from hypoxia. To do this either bypass surgery or – as in this case – stenting of the excretory part of the right ventricle of the heart is performed. 

 

- The tetralogy Fallot is a common vice. But this is the first child in SPbSPMU who underwent a radical correction after stenting, – explained Andrey Nokhrin, head of the cardiac surgery service at SPbSPMU.  

He also added that in low-weight children the use of a modified Blalock-Taussig shunt is associated with a high risk: mortality after palliative surgery of the first stage reaches 20-30%. Therefore, the method of choice is stenting. 

 

- This manipulation can be performed in children with a weight of 1.5 kg. This child had a small weight and heavy oxygen dependence, he had recurrent spasms of the excretory part of the right ventricle. Therefore, such a tactic was chosen, – said Andrey Nokhrin.

Thanks to the minimally invasive palliative surgery, the child was able to gain strength before a complex and lengthy surgical intervention. In mid-December, when the girl was five months old, and her weight was close to five kilograms, a radical correction of the heart defect became possible. The doctors removed the previously installed stent, reconstructed the outflow pathways from the right ventricle, and closed the ventricular septal defect. 


- The postoperative period of the child was normal, without any special features. Yesterday the patient was transferred to a normal ward, – said the head of the Department of anesthesiology-resuscitation for children with cardiac pathology Yevgeny Trizna

 

 


- The prognosis for this child is favorable. It is likely that the girl will not lag behind her peers in physical development. But here a lot depends on what kind of rehabilitation she will receive after the operation. A good example is the snowboarder, two-time Olympic champion Shaun White. In his childhood, he was also found to have tetralogy of Fallot and had two operations before the age of one. Nevertheless, he achieved serious success in sports, – Andrey Nokhrin told.
